What do I need to send for a schedule request?

If you wish to make a schedule request, please:

1. Click on the e-mail link below
2. Type schedule request in the subject line
3. Include parent's name and address, child's name and age, diagnosis (if known), Therapy needed (OT and/or ST) Include frequency requested (1 time per week, 2 times per week, etc.) – Please note final frequency determination will be determined by the therapists at Kiddos’ Clubhouse
4. Listing of top 3 choices of day and time for therapy to occur
5. Phone number where you can be reached
